Due to recent laws passed in order to protect privacy ( especially in the health space) we can only send PHYSICAL MAIL with your permission. By filling out this form you are giving us consent to send physical items through regular mail.
Examples of physical items we may send in the future include but are not limited to POSTCARDS, FLOWERS and MS GYM SWAG.
